You do not need this form. Log in to the
client portal to view records
and print vaccine certificates yourself, any time, no waiting on us.
Use this form to move records between practices: to have us request them from your previous veterinarian, or to have us send your pet's records to another clinic, a specialist, a boarder, or an insurer.
How long it takes
- Most requests are handled within two business days.
- Remember we are closed Thursdays, and Ridgecrest is open Mondays and Fridays only.
- If a specialist needs records today for an appointment today, call (760) 373-7387 instead of using this form. We will fax or email them directly.
